This is where the terminology gets confusing, because "screw-back" means two completely different things depending on who's using it.
In fashion jewelry, a screw-back earring is a traditional stud: the decorative front and the post are one piece, you push the post through from the front of your ear, and you twist a small back onto the post from behind. The back is the part that screws. It's a butterfly back with threads — more secure, but it still sits raised behind your lobe.
